Many communities are program-rich but system-poor—offering numerous well-intentioned services.  Yet many lack the coordination, accessibility, and sustainability necessary to truly support individuals, especially those living at the intersection of disability and poverty. In these fragmented landscapes, even abundant resources often fail to reach the people who need them most. Systemic gaps highlight the needed supports that don’t just respond to need—but prevent the conditions that create crisis in the first place. What can educators, service providers, policymakers, and community members do to shift from isolated efforts to integrated systems that proactively reduce the need for resilience?  What strategies and policies can be implemented to build coherent, long-term support structures? How can data-sharing, resource coordination, and cross-sector collaboration break down silos and ensure more equitable access to education, employment, and community life for individuals with disabilities?
