Access to the Affordable Care Act: A citizen's right or a privilege?

According to the Kaiser Family Foundation, despite the passage of the Affordable Care Act, 5% of Americans remain uninsured, while 32% now have an account in collection for medical debt. Is access to healthcare a basic human right or a privilege only for those who can afford it? How does the U.S. compare with other developed countries in this respect? Should the US move toward universal access to health care based on medical need? Where does “end-of-life care” fit in when the most difficult and expensive decisions are looming? What role do personal responsibility and accountability play in health care?
