Sustainable healthcare and insurance in the U.S.

Many believe the United States has maintained high quality healthcare with rapidly escalating insurance premiums associated with the Affordable Health Care Act. Meanwhile, congressional and executive leaders have sought to repeal/replace the act.  What should an improvement of the U.S. health care system look like? Are the alternatives any more sustainable or accessible than the status quo? What is our responsibility as individuals, small groups and communities in mitigating the surging costs?
